MEDICAID RECERTIFICATION
Medicaid recipients are required to recertify their Medicaid annually. Medicaid recertification is needed to demonstrate that the recipient still meets income and asset eligibility. As part of our services, we work with our clients to make sure all of the proper paperwork is completed and then we hand deliver the recertification form to the appropriate Medicaid office.
ASSIST WITH PLACING AIDE SERVICES FROM HOME CARE AGENCIES
Working with Managed Long-Term Care (MLTC), we assist families and clients in choosing a home care agency that works best for them, while helping them secure an aide as soon as one is needed. Our relationships with agencies can help arrange for an aide even before Medicaid is approved.
POOLED INCOME TRUST
For clients that receive community medicaid and earn over $875 a month, we help them protect their income through a Pooled Income Trust. The trust is set up in the name of the Community Medicaid recipient with a trust company licensed by the State of New York and approved by Medicaid. The funds from the trust are used for non-medical expenses in the name of the beneficiary of the trust.
MAXIMUS AND MANAGED LONG TERM CARE ASSISTANCE
When a client is approved for Medicaid we coordinate the Maximus appointment, which assesses to see if the client qualifies for home care. Once the Maximus appointment is complete, a Managed Long Term Care (MLTC) is selected. We work with clients and families to advocate on their behalf so they can receive the hours of home care assistance to fit their needs.
